General Consent Form – Seasonal Camps

I consent to photographs / and or video recordings of my child to help promote the Antrim School of Music. 

I will allow these images and recordings to be shared on the Antrim School of Music and social media platforms for promotional purposes only. 

I give consent for my child to be treated by a First Aider in the event of an incident. 

I adhere to notifying the facilitators of any medical conditions that my child has, and that my child should self-medicate where possible. If my child needs assistance to administer medication e.g. epi-pen, I will inform the first aider who will supervise all medication and give permission to administer when necessary. 

I understand that in accordance with new GDPR Legislation, my details will be processed and held in accordance with these regulations, purely for administrative purposes.
